first thought.....heater burn.....then realised no heater in the tank.

2nd thought cauliflower disease. But from memory cauliflower disease doesn't eat a hole before starting out and doesn't quite look like what i can see......

This seems to be a hole in the upper lip area with the roughness coming out of the hole or either parts not eaten away by whatever has done the damage.

Any suggestions greatly appreciated.

sorry not sure, could be fungal or bacterial but does not look to have the redness many bacterial infections get. I would treat with some pimafix asap or other general anti fungal if you have any? something has eaten the flesh away.
also looks to be a sore or ulcer at the side of the mouth?
what else is in the tank? could a fight be possible, perhaps he has been bitten?
first do a big water change as water quality is vital for fish recovery, as I am sure you know.

tankmates are one other choc pleco and some jaguar fry. It's in the 4x2x2, a bit of gravel no ornaments. None of the other fish seem infected by it and are eating well.
